I'm a female with a small heart shaped face, narrow small chin & mild underbite. My chin was set back about 10mm from where it should be & is quite short vertically. I have a med anatomical implant which gave 'moderate' improvement to the profile but it's widened my face & my chin looks square & masculine. Mentolabial fold has also been over-accentuated. It doesn't work! What implant could be used to give a subtle vertical increase, good projection and NO lateral width? Genio is not an option.

Dear Littlehiphop, There are many different chin implants that may be used that will create a more feminine appearance. This will be determined upon consultation and examination. Implants are designed in many styles and shapes so that they best fit each patients facial aesthetics and some are designed without lateral width for patients like yourself. An examination is necessary to select the proper chin implant. There are hundreds of sizes and shapes which can be modified to meet the needs of each individual patient.

I have performed many Chin Implant surgeries for over 25 years.  A non-smilling profile and front photo would be the best for evaluation.  If you've had a Medium anatomical implant (what's the implant manufacturer as the dimensions and shapes are not the same) and seek a different look, there are a couple of considerations that I feel you should understand. There is no way to extend the vertical aspect of the chin using a Chin Implant.  Doing so would create a ledge along the lower aspect of the chin as the implant extends vertically.  The Chin Implant must remain at a horizontal placement along the anterior (front) segment of the chin.If you remove the lateral, tapering aspect of any Chin Implant, the result is an un-natural "Pharaoh" look to the chin which is most un-attractive. The mental crease should be approximately 5 mm's deep and will deepen with a chin implant that's placed in the proper anatomical position.For those reasons, I believe your best option is to consider some other version of the anatomical chin implant selected to meet some of your needs using an incision below the chin and not an intra-oral approach.
