What is the best course of action for intramuscular gluteal implants that have displaced (broken through)?

I had oval 550cc cohesive gel implants placed in June of 2019. In October of 2019, I took a long road trip and sat in a reclined position. After about 6 hours my glutes were in pain and I had a fever for days. An MRI recently indicated that the implants are in perfect condition but have migrated to above the muscle. They broke through the pocket. Should I have them removed? Or put back (intramuscular) in and somehow reinforce the pocket? What does the literature indicate as the best way to fix?

Hello, thank you for your question. It is very unusual for the implant to migrate, I recommend you to schedule a consultation with a board certified plastic surgeon. Remove your implants and they have to be place intramuscular.

Buttock implants that were properly placed into the intramuscular tissue plane do not migrate, displace, or break through the muscle into the subcutaneous tissue plane at five months after surgery, especially not both sides symmetrically. I have heard this very similar story/concern many times before from other patients, so I am guessing that they were probably never actually put in the muscle and rather they were always on top of the muscle.  As they settled into place and the swelling went down over five months, it became evident that they were indeed on top of the muscle - this is the most likely story. That all being said, what needs to happen now is for them to be removed from the subcutaneous pocket and repositioned into the intramuscular pocket. This can usually be done all in one surgery. You may also want to consider changing to a more form-stable and anatomic shaped implant such a Stanton Anatomic gluteal implant which will also help avoid the gumdrop appearance that round and silicone gel implants create while bending over.
