I'm not sure whether to get upper jaw surgery and a sliding genio or to go for upper jaw surgery alone with anti clockwise rotation of the lower jaw to fix my lip incompetence and chin.

It's extremely difficult to decide on what procedure/procedures are required based on this single photo. However, from what I can see in the above photo is a deficient mandible and mild excess in maxilla. Based on dental occlusion, a sliding genioplasty or bilateral sagital split of mandible can solve this problem. Good luck to you.
