Is It necessary to extract four bicuspids to reduce protruding teeth and lip incompetence?

I am 24 and have an open bite, lip incompetence, and protruding top and lower teeth. I am currently on braces and was informed I would need to extract 4 bicuspids before jaw surgery.

This is best answered by your surgeon. It depends on the type and extend of the surgery. Usually, I would like my patients to stay in town for one month.

Orthognathic surgery or should I have orthodontic with TAD to correct overbite with incompetent lips?

Orthodontic treatment should not be  used to replace jaw surgery. It might correct your bite but your profile might suffer. Please see a board certified orthodontist.
