IPL for olive toned skin will make it worse & it did on me.Peels, ok which one? Vi, melange, cosmelan, tca? (obagi creams are harsh/expensive)Maintence?custom hydrquinone/Kojic/azeliac/ retin a/arbutin mix or inhibitors? I use sunscreen daily& tinted my car.Orignially from hormone meds & mostly under eyes/cheeks.The laser also caused capillaries under eyes & nose.What can help that too?I tried a YAG/Ktp laser 3yrs ago & face swelled like a balloon,it hurt & didn't work/plus trauma created more.

Answer: Melasma, lasers and broken capillaries. Melasma can not be completely cured, however effective treatment are possible. This is because your skin is extremely sensitive to UV and even the smallest amount can stimulate your pigment cells to produce colour. The mainstay of treatment is strict UV protection- hats, sunglasses and SPF every 4 hours. I prefer to use a combination of creams and laser for melasma. Hydroquinone can be used (5-8%), along with vitamin A creams. I usually start my patients on laser (low dose Q switch or Picosure) a few weeks after they commence on creams. In some cases I combine glycolic AHA peels, and in other cases I use a tablet called Tranexamic acid to help.As for broken capillaries, V Beam or Excel V can help, but the parameters can be tricky in darker skin types, - I suggest you seek the advice of a Board certified Dermatologist or Plastics for this.
