You appear to be a candidate for jaw surgery. However, more often than not, jaw surgery requires both upper and lower jaws to be moved. Your lower jaw is quite recessed, and a large advancement usually means the top must also be moved as well to keep your bite aligned. Moving the upper jaw will also give you more support in the maxilla and cheek area. There are many things to consider when thinking about jaw surgery, but the most important is better quality of life. With that comes the aesthetic benefits as well. Depending on the advancement, you may or not need a genioplasty. Luckily it is fairly easy to add on.
