I went to a dermatologist for my hair loss which has gotten worse in the last two years. I’m 40 years old. He said no to a biopsy and things rogaine and spirolactone don’t have high success rates. All of the 17 labs I had done were normal. He said he thought I might have both FPHL and CTE. I had 3 surgeries in 2015. I loose about 50-70 day. Can you tell what kind of loss I’m looking at? And is he right is rogaine and spirolactone not successful? I think he called my hair loss diffuse thinning.

Answer: CTE or FPHL You might have both simultaneously, or reaction from your surgeries, and a biopsy would not likely differentiate the two. HairCheck and hair counts may help. Those therapies DO have a high success rate and may help significantly if you use them consistently (and the spironolactone usually requires a high dose). Also consider low level light (the helmets) and off-lable oral minoxidil. Bottom line is an internet doc probably cannot tell, so find someone who is interested in your type of hair loss that you trust. The earlier you start on treatment the better your results, too. Answer: Female genetic hair loss vs CTE I never treat women like you with hair transplants when there is diffuse thinning present because it does not produce value for you and certainly does not reverse the hair loss. The diagnosis sounds like typical female genetic balding and if I am correct, your donor area will show significant miniaturization, which always makes the diagnosis for me.
