Fat removal from forehead - can this be removed with a brow lift at the hairline? (Photo)

Question

I had asked this question but I want to be more specific. I had a fat transfer to my forehead about 18 years ago because I had a slight indent across my forehead due to a stronger brow bone. It looked great at first but then as it died it left some lumpiness with the most noticeable right in the center of my forehead a little larger than a quarter . When I press on it it's soft, not hard. I would like to get a browlift at the hairline although I don't have a large forehead. I don't care if the hairline is s little low but can they remove that center mass or improve it. Thank you

Yes, I recommend a browlift done at the hairline.  This will allow the access to get down to the area in question and the fat can be excised directly.  This will also lower the hairline slightly.

Based on your description, if the fullness represents retained fat from your previous fat grafting, it may be possible to reduce or contour it during a hairline brow lift. Depending on the location and tissue characteristics, your surgeon may perform direct excision or conservative contouring to improve the forehead shape. An in-person examination is necessary to determine the safest and most effective approach.

Hi mickey458, thank you for the detail, it helps. What you describe sounds like an old fat graft that partially resorbed and left a soft area of irregular fullness, which is a recognised long-term issue with fat transfer to the forehead. A hairline, or pretrichial, brow lift does open a plane across the forehead, so in principle a surgeon can contour or remove soft tissue at the same time. Whether it actually reaches your particular lump depends on how deep the nodule sits and exactly where it is relative to that dissection plane. A soft central-forehead nodule can sometimes be dealt with directly through a small, well-placed incision instead, if raising your brow is not something you want for its own sake. So the honest answer is that combining the two is often possible but not automatic, and I would not promise it from a photograph. It needs examination, feeling the lump and judging its depth, before deciding between a hairline lift and a more direct approach. I operate only on faces, including the forehead and brow, and would want to assess this in person.
