HSV 1 scar removal treatment options? (Photo)

Question

Hi, What can be done to remove a "below the surface HSV 1 lesion"? It has never formed into a blister, but it's been there a while (6 mos+). A plastic surgeon gave me 2 high dose pills of valtrex, but it did nothing. I'm Looking for something scar free, and fast treatment with no down time. Thanks in advance.

At least from the story and from the appearance in the photo, it is quite possible that the bump is fibrosis, I.e. scar tissue from the antecedent active herpes infection, which is not a particularly unusual sequel. When no active herpes infection is present, it is no surprise that the Valtrex would have no effect. And, if this is indeed scar tissue, the bump is likely permanent unless some appropriate treatment is undertaken. The use of an intralesional injection of an appropriate anti-inflammatory agent, in the proper concentration and amount would be a very reasonable approach in this case. This must be done with appropriate caution, however, especially in patients of skin of color, to avoid causing hypopigmentation. You would be wise to consult with an experienced, board certified cosmetic dermatologist and best of luck.
