At least from the story and from the appearance in the photo, it is quite possible that the bump is fibrosis, I.e. scar tissue from the antecedent active herpes infection, which is not a particularly unusual sequel. When no active herpes infection is present, it is no surprise that the Valtrex would have no effect. And, if this is indeed scar tissue, the bump is likely permanent unless some appropriate treatment is undertaken. The use of an intralesional injection of an appropriate anti-inflammatory agent, in the proper concentration and amount would be a very reasonable approach in this case. This must be done with appropriate caution, however, especially in patients of skin of color, to avoid causing hypopigmentation.
