You are showing profile changes, based on the change in the lips, that only bimaxillary orthognathic surgery can accomplish. It is the teeth position that affects the position of the kids not the bone. But whether you have the occlusion that can support that type of surgery and the expense and recovery that goes with that type of facial bone surgery is what you are willing to do are issues to be considered. If one is willing to pass on the profile lips changes the other facial changes  can be achieved by custom midface and jaw augmentation implants.
