I’d like to be a C cup if that’s possible. The 1275 is a requirement of my insurance company. I’m trying to find the most cost efficient way to have this done because the back, neck and shoulder pain have gotten to be unbearable and are restricting my activities. I’ve had a consult with a local surgeon and he said he could not commit to removing this large amount. I just want another opinion before paying for another consult.

Thanks for your question and I am sorry you are suffering uncomfortable symptoms due to your breast size. The good news is you appear to be a very good candidate for breast reduction, although the efficacy and safety of removing that much breast tissue is impossible to evaluate without an in-person examination. Most important is to work with a board-certified plastic surgeon that specializes in breast reduction to determine how much skin and tissue needs to be removed to create a proportionate look and attractive breast shape. I do many large breast reductions, and in most instances someone presenting with your size will end up somewhere in a D range with a standard technique unless you do a "free nipple graft" technique, which is rarely indicated. Also keep in mind that an over-resected breast reduction looks out of balance on larger frame patients.The amount of tissue you have to leave to keep circulation to the nipple effectively limits how small you can make a patient. Likewise, patient's with larger torsos end up in those cup sizes just because of the way a bra fits circumferentially. It is obvious that you have macromastia (large breasts), and it is no surprise that you are having the symptoms that you describe.  The difficult thing in your case is the requirement to remove 1275 grams of breast tissue per side.  That is a very large reduction, and based on your pictures alone which of course do not substitute for an in-person physical exam, I doubt I would be able to remove that much yet leave you with healthy remaining tissue and give you a nicely shaped breast.

Looking at your photos I might say that you´ll be a great candidate to get a breast reduction because you have enough tissue to reshape to reduce and lift it. Before all this, of course, you need to get a complete medical evaluation and laboratory exams to check everything is fine. Also please try to find a board certified plastic surgeon, remember that your safety, always, is going to be first.

your procedure must meet that or you will be responsible for all the costs (OR, anesthesia, surgeon fee).  Reduction outcomes cannot be predicted but you are guaranteed that you will be smaller.  If your surgeon cannot guarantee the minimums can be removed, then you are probably on the light side for what is required.  You can either choose to go forward with insurance coverage and accept whatever outcome arises from removing almost 1300 grams, or you can have the procedure done at your surgeon's discretion and if 1275 gm cannot be reached, understand you will be paying for your procedure,.. and often most practices require this up front if you choose this option.
