I had surgery two or three years ago; that bump was present from the moment I took the bandages off my nose, and ever since then, I’ve constantly felt that it’s there, along with a sensation of tightness.

Hello and thank you for your question. I don’t recommend having rasping performed under local anesthesia. This will usually make the nose look better for 1-2 months and then it will actually get much worse long-term afterwards, with contour irregularities developing. This is because the skin of the dorsum is extremely thin. A much better approach is to do a formal revision with rasping and placement of a dorsal onlay fascial graft to help smooth the dorsum. Make sure you are going to a surgeon with hundreds of long-term before and after pictures on their website with standardized lighting and standardized backgrounds. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

A bump that has been there since the splint came off and has not changed in two to three years is unlikely to be swelling. It is more often structural: an edge of bone left after the nasal bones were narrowed, a cartilage irregularity, or a graft that has shifted, and the tight feeling can come from scar tissue over it. Which of these it is matters, because a small bony edge can sometimes be smoothed with a limited procedure, while a cartilage problem may need a more formal revision. A careful in-person exam, sometimes with imaging, by a surgeon who does revision rhinoplasty is the sensible way to find out.
