I scrated my penis while having sex with my girlfriend, nail jammed into shaft creating a puncture wound. a My penis has healed, however, a sizeable pigmented scar is left and is a quite noticableraised bump almost the size of a B-B remains.

A puncture wound from a fingernail can sometimes heal with a raised, pigmented scar, especially in an area where the skin is thin and subject to friction. Based on your description, it sounds like you may have developed either a hypertrophic scar (a thickened scar that stays within the boundaries of the injury) or a small scar nodule with post-inflammatory hyperpigmentation.

The first thing I would want to know is how long ago the injury occurred. Scars continue to remodel for many months, and it is not uncommon for them to gradually flatten and lighten over time. If the injury is relatively recent, patience alone may result in noticeable improvement.

For a persistent raised scar, silicone scar gel or silicone scar sheets can sometimes help soften and flatten the area when used consistently over several months. Because the skin of the penis is delicate, I generally recommend avoiding harsh over-the-counter scar treatments, acids, or bleaching products unless specifically directed by a physician.

If the bump has remained unchanged for many months, is firm, or is causing cosmetic concern, there are in-office treatments that may help. Depending on the type of scar, options can include steroid injections to flatten thickened scar tissue, laser treatments to improve pigmentation and texture, or, in select cases, minor surgical revision.

One important consideration is making sure this is truly scar tissue and not another type of growth that happened to appear after the injury. If the lesion continues to enlarge, becomes painful, bleeds, or changes in appearance, it should be evaluated in person.

The good news is that scars from this type of injury are usually benign, and there are several treatment options available if the appearance remains bothersome after the scar has fully matured.
