My mid and lower face is heavy and beginning to sag. I feel like this ages me quite a bit and it is my biggest insecurity. I am a 32 yo female, 5'5" 130 lbs. What treatment would be best to remove this stubborn pocket of fat? Would I be a good candidate or buccal fat removal? Maybe liposuction?

Weight, age, possible previous treatments should be taking into consideration as well. Based on your information provided, it seems that there is an unusual lower lateral fullness and some of the midface fullness seems to have descended as well to some degree. The best possible option is to see at least one plastic surgeon, whose focus is the face to have an in person consultation, to discuss your goals and possible solutions. Best of luck

Answer: Liposuction vs. direct fat shaping using face lift approach This is an interesting fat distribution along lower cheeks and jawline. I do not think that buccal fat removal would create significant difference, if any? Most likely that superficial liposuction of jawline and neck would create some improvement, since it has to be conservative to avoid nerve injury. My preference would be approach for lower face/neck lift (in front and behind ear scars as for typical face lift) which will give an opportunity to precisely address shaping of the fat along jawline, lower face and neck. In person consultation with a board certified plastic surgeon, expert in face lifts, is recommended. Good luck.
