Heavy/sagging lower face. Wondering if buccal fat removal would be a good option? (Photo)

My mid and lower face is heavy and beginning to sag. I feel like this ages me quite a bit and it is my biggest insecurity. I am a 32 yo female, 5'5" 130 lbs. What treatment would be best to remove this stubborn pocket of fat? Would I be a good candidate or buccal fat removal? Maybe liposuction?

Hello, and thank you for your question. Those with a larger face with extended roundness in the mid-to-lower face but with significant bone structure make good candidates for buccal fat removal. Partial removal of fat pads can help enhance cheek definition and achieve slimmer facial contours along the jawline. Based on your photos, you do appear to have fullness in your lower face.

To treat sagging skin, there are skin-rejuvenating procedures to give the face a lift. Combining fillers with Botox can create a non-surgical facelift, while lasers and Profound RF (microneedling combined with RF energy) can boost collagen to help renew and tighten the skin.

thank you for posting your pictures. It is somewhat difficult to make a good judgment based on the limited information and the pictures displayed. Weight, age,  possible previous treatments should be taking into consideration as well. Based on your information provided, it seems that there is an unusual lower lateral fullness and some of the midface fullness seems to have descended as well to some degree. The best possible option is to see at least one plastic surgeon,  whose focus is the face to have an in person consultation, to discuss your goals and possible solutions. Best of luck

This is an interesting fat distribution along lower cheeks and jawline. I do not think that buccal fat removal would create significant difference, if any? Most likely that superficial liposuction of jawline and neck would create some improvement, since it has to be conservative to avoid nerve injury. My preference would be approach for lower face/neck lift (in front and behind ear scars as for typical face lift) which will give an opportunity to precisely address shaping of the fat along jawline, lower face and neck. In person consultation with a board certified plastic surgeon, expert in face lifts, is recommended. Good luck.
