Question

I had two FUT before only for frontal hairline. The first one was a failure . The second FUT was done with 1800 grafts. Now, will there be issues with tightness for third FUT? are there any downsides ?

It does appear that you have sufficient donor hair density to perform another procedure.  If tightness is an issue FUE harvest can be performed

I plan to perform a FUE operation for you with  2200-2300 grafts. skin elasticity  is  the crucial  factor  for  strip  excision  which  I dont recommend  you  as  third  operation . There will be almost no scar or obvious marks nor at the donor area neither at transplantation site with FUE tech. But ıf you shave your hair with razor, you or others may only realize a tiny white dots on your skin when looked very closely..They are very tiny and invisible scars of punch insertions . Actually I have no complaint about those scars before…

You likely have enough for 3000 grafts but may want to consider FUE if there are issues with tightness. Whether or not you should use of 3000 grafts depends also on your current age and how much balding you and your doctors think you have to go. Finally, all patients with failed procedures should consider having a scalp biopsy to rule out autoimmune  and other issues before moving forward again.

If the scalp tissue is too tight it may make the wound more challenging to close. It may be better to consider a FUE procedure if you have already had 2 FUT procedures in the past.
