My surgeon told me that my nipple will return to normal and smooth out after some time, I can see that it looks like he pulled it all to the center and folded my nipple inward then stitched it up, I am having that bunching feeling and I feel the need to try to stretch the nipple smooth, is this normal? This is 8 days after surgery plus the incision looks like a 5 year old tried to put it together

Things often look very strange right after surgery and at 3 months they'll look perfect.  It's not uncommon for patients to think a pre-schooler stitched them in the early days after surgery.  This shape and puckering will soften and smooth out.  It's too early for you to get concerned about the final result.

The swelling distorts the appearance of the nipples and areolas early on, and they will go through a lot of changes in the first several weeks after surgery. Way too early to tell but I know a lot of surgeons rely on patients not looking at things for several months before critiquing which is almost impossible.  It will improve (or should), but if it doesn't you should be open and upfront with your surgeon about your feelings.  Let things heal and give your surgeon the opportunity to make things better.

Nipple puckering is normal at a days after a breast lift.  With time the tissues should straighten out stretch out and become normal.
