Answer: Correction of columella Based on this photo, best treatment would involve surgical correction of lower lateral cartilages. It will require small incision across columella and reshaping of medial crura with sutures. It can be done in local anesthesia. Good luck.

The columellar shape that you are referring to is caused by malposition of the lower cartilage structures that support the tip of the nose. I recommend an open tip plasty with manipulation and support of the columella. The procedure has one small incision and a very easy recovery. It is a procedure that can easily be done at a private office based surgical facility. I would place a spint on the nose for 4 days and there would be no packing makin the recovery easy.
