Mid 20s F, ~5 feet tall and 130 lb I often get mistakened for older than I am and I feel I do look masculine in some ways (I had PCOS growing up that I’ve only recently started taking medication for) I am interested in upper jaw shortening surgery with lower jaw if needed I also have TMJ ithat flares often and would be nice to stop taking ibuprofen

Surgery for people with a long midface (often referred to as long-face syndrome) is optimally treated with jaw surgery (orthognathic surgery). This condition is typically associated with a gummy smile, an inability to close the lips without straining the chin muscles (a condition known as lip incompetence), and may be related to a small lower jaw. 
It is difficult to fully diagnose your condition from the single photo that you have posted. This surgery is optimally performed by an expert surgeon with many years of experience with a practice limited exclusively to orthognathic surgery.
