Weak chin and jaw, chin implant or genioplasty? 24 yo and unsatisfied with my chin (seems retrognathic) (photos)

Dear all, I had orthodontic treatment 5y ago, they advanced my lower jaw with an apparatus but the effect has gone away. Lately I saw a doctor who told me I wasnt retrognathic&my bite was ok. Still, I can see in the mirror that sth is wrong. I read the implants do not really increase the height of the chin&I feel the height is an issue too as my nose seems long. Also, they have a risk of infection, misplacement&bone atrophy, but how big is it? But genioplasty can have bad effects, too.

from your photos you do appear to be "retrogenic" which means your chin projection is reduced. This appearance may be improved with either a genioplasty or a chin implant. In my opinion a genioplasty will give you a more natural result which will be more stable in the long run. The procedure is done via a small incision inside the lower lip and has a very small risk of complications. I would suggest you meet with an experienced craniomaxillofacial surgeon to discuss your options.

Thank you for your question and photos. I am happy to hear that your bite is still in good occlusion. The issues that you describe can best be addressed with a sliding genioplasty. As you stated, a chin implant does a poor job of changing the vertical and really only increases projection. The comment about your nose being too long is a very common one among patients seeking chin surgery. The lower face greatly impacts how the upper and midface is perceived. In-office facial measurements will help determine exactly how much you need to be moved forward and changed vertically in order to obtain optimal facial balance. I tend to lean toward a sliding genioplasty in candidates as yourself for many reasons. There is always a risk of infection with any surgery, however without the addition of a silicone implant, the risk of long term problems significantly decreases. It will be beneficial for you to ask your board certified plastic surgeon if they can create computer imaging to give you an idea of what the expected outcome from surgery will be. This often helps both parties be on the same page in terms of the results you are seeking. I wish you the best of luck!
