3 years post-op- Persistent swelling and redness after cyst and lipoma removal a few years back. What could this be?

I had a pilonidal cyst back in 2018, that first showed 2 weeks before my period. It progressed so quickly, the next week i had to have it drained. After a month of dressing the wound it healed. A year and some change later, i returned to the doctor’s because the site would swell and become painful to the point that I’d have to lay on my stomach. The determined that a lipoma had formed and removed it. within 2 weeks the scar healed. Now in 2022, it is extremely swollen and red. What can this be?

I'm sorry to hear about your lipoma returning. Unfortunately that is one of the highest risks of removal as any small piece of the lipoma left behind can ultimately grow back. A lipoma typically encapsulates itself, meaning it stays as one contiguous piece, however if it is in multiple pieces it...
