The photo shows central abdominal fullness with loose skin both above and below the belly button, and there may be some horizontal looseness toward the midline. A fleur-de-lis tummy tuck can be very helpful when there is significant side-to-side skin excess that a standard low horizontal tummy tuck cannot tighten well. The tradeoff is a permanent vertical scar in addition to the lower scar. If most of your looseness is mainly below the belly button, a standard or extended tummy tuck may be enough. If the upper abdomen and waist area have a lot of horizontal laxity on a pinch test, FDL may give a flatter and tighter shape. The best choice requires an in-person exam, because the photo cannot show skin quality, internal fat, muscle separation, or how much skin can safely be removed.
