How to fix crooked gluteal cleft / sacral dimple? (photo)

Although no guarantees, it may be possible to centralize your gluteal cleft but will definitely first require a consultation with a board certified plastic surgeon (preferably one specializing in buttock implants as this region is familiar for making the incision and dissection). Liposuction and/or surgical transposition of the cleft are two option that come to mind of hand. Glad to help...RAS

A tough problem. The dimple could be nothing, or signify a cyst. It may be associted with chronic constipation.  Unlikely that a non-surgical approach is possible. Also, surgery may not be a good option.  An in-person exam is needed.   

The crooked gluteal fold seems to be caused by more fat on one side than the other. In person evaluation is needed. If too much fat it can be repaired by liposuction and fat transfer to the gluteal dimple.Samir Shureih MD. FACSsshureih@msn.com
