Without the benefit of a clinical exam, it appears that you would be a candidate for submental and jawline liposuction. In addition, I sometimes perform a platysmaplasty (tightening of the muscles) and remove a second layer of fat beneath the muscle. Both these procedures are performed in the office with sedation anesthesia.

It would be difficult to give you accurate evaluation without in person exam. Fillers, Bottox and, in particular, tread lift, will not properly address this problem. The only appropriate procedure would be anterior neck lift which will address issues of deep fat underneath the muscle, chin ligaments and platysma muscle. In person consultation with a board certified plastic surgeon with experience in this type of neck lift (a small incision underneath the chin), is recommended. Good luck.

Much more information is needed, such as your age, and a full set of facial and neck photographs from all angles with your hair pulled back behind your ears. From the limited photographs it appears that you have significant genetic fat deposits located below the platysma muscle which is going to require a neck lift procedure. This involves liposuction to remove the fat deposits above the platysma muscle, and surgical extraction of the fat deposits below the platysma muscle which also includes a platysma plasty to significantly improve the jawline. The procedure as performed under general anesthesia, and you should anticipate two weeks of visible bruising and swelling. Full healing is several months.
