Is this a fat graft protrusion? How can this be fixed? (Photo)

Hi! I am 30F, diagnosed with craniofacial microsomia. I have had a couple of surgeries done on my nose and am overall happy with the shape, but have noticed a weird bulge/pocket when i make certain expressions. i wasnt given much info on what was done as all of my surgeries were covered under Canada’s public healthcare, but i did have cartilage grafting during my last procedure and fat grafting done to the pictured area at age 17. Ive gained about 20 pounds since that procedure so i think that that may be the cause of what im seeing, especially since you can also see what I believe to be the injection scar next to it (blackhead looking thing). My nightmare scenario is that there was cartilage grafted to that area which is now poking through the skin, any insight is appreciated— i really wish i knew more about the techniques which were used.
