In California AB 1621, passed in 1998 makes it a criminal offense for initial denial of coverage for removal of this congenital abnormality by other than a physician in the field that specializes in the care of this condition. The law defined cosmetic vs reconstructive surgery and this is clearly in the reconstructive category as it is both congenital and not a normal structure of the eye. The bill was needed because insurance companies were denying coverage for things such as lack of an ear at birth, microtia, or cleft lip.
In this case you would want to see an ophthalmologist with training in plastic surgery, an oculoplastic surgeon. Other specialties might be involved to fully diagnose and plan treatment.
As far as the NHS is concerned, they should have an ombudsman you can present your case to, or have your local MP intercede for you. A copy of our California law might enlighten them.

It can be removed by an Oculoplastic surgeon or a facial plastic surgeon. It is important to know if the lacrimal gland is involved or not. Also, if the lipoma can be approached through the skin.
