I am 23 and one eyebrow is always lower than the other. I have tried botox to fix this but nothing seems to work. Are there any alternative options?

You could benefit from an endoscopic brow lift.

A brow lift I s a cosmetic procedure that aims to address the sagging and furrowing of muscles and tissues in the forehead area, which can create an angry or tired appearance. This technique involves selectively releasing these muscles and tissues to smooth out horizontal furrows and frown lines, resulting in a more youthful and refreshed appearance. Additionally, the procedure can elevate the eyebrows in a natural and glamorous way, which can also reduce the appearance of sagging upper eyelids.

I personally prefer the ENDOSCOPIC BROW LIFT technique because it can achieve the desired results with small incisions that are concealed within the hair. Unlike most plastic surgeons who use five incisions, I use only four incisions and believe that this approach allows me to achieve superior outcomes in terms of brow elevation, precision in shaping the final brow contour, and the longevity of the results.

Of course, an « in person » exam is necessary. I recommend to consult with a skilled board certified plastic surgeon for the best results.

The photo provided demonstrates that the tail (so called "lateral aspect") of the right eyebrow is flatter than the left. There are multiple ways to reposition this portion of the eyebrow e.g. using non surgical techniques such as botox and threads. More predictable results can be achieved with surgery. There are many surgical techniques to lift the brow.
A facial plastic surgeon can make a thorough assessment and advise which technique would benefit you the most.

While you don’t appear to be a candidate for brow lift surgery at this time, you may consider combining Botox treatments with dermal filler injections to create a more symmetrical appearance. At least from the photo provided, the problem seems to be a rather straightforward and easy fix problem with the use an appropriate filler to help support the and elevate the outer right eyebrow. As we age, our eyelids naturally sag and droop. A combination approach would be best to see visible improvement. In terms of non-surgical treatment options, I would recommend a Chemical Brow Lift (using Botox, Dysport, Xeomin, or Jeuveau) and filler injections to rejuvenate the temples, eyebrow, eyelid, eye troughs, upper eye hollows, and forehead. I typically combine this with Ultherapy and thread lifting to lift the brow; RF and skin tightening laser treatments to stimulate collagen production and rejuvenate the skin; and laser or superficial RF devices for improvements in texture, color, and any crepey skin around the eyes as needed.
