Asymmetrical eyes and scleral show - what surgery will fix this? (Photos)

I am a 25 year old male. My eyes are my biggest insecurity as they make my face look lopsided and one side looks like its sagging downwards giving a melting face look. Now I have had Bell’s palsy before which might have contributed to this facial asymmetry but I also seem to have a negative orbital vector and my right cheekbone is less prominent than the left which may explain why it droops more. What type of surgery might be able to fix this issue? I have attached some pictures to show the problem

It is very likely that correction of the upper eyelids ptosis will significantly approve your orbital asymmetry without any additional surgery. Do ptosis correction first. Good luck.

Based on this photo and without preoperative photos to compare, you seem to have very good result after lower blepharoplasty and fat grafting. Asymmetry is so minimal that can be easily corrected with skillful make up. Good luck.

Based on these photos it appears that a lateral canthopexy on the “lower” eye would create more symmetry. In person consultation with a board certified plastic or oculoplastic surgeon is recommended. Good luck.

Based on these very limited photos which do not show entire periorbital area, it does not look like you should expect much of the improvement after lower or upper blepharoplasty. In person consultation is recommended.

Thank you for your photo. I believe you should first have your eyelids checked for ptosis to be sure there isn’t an underlying abnormality with your levator muscle. A mid facelift can tighten the skin around the orbits to smooth out some of the contour irregularities. A lower blepharoplasty can be done to transpose fat to hide the tear troughs. Best Wishes, Gary Horndeski, M.D.
