By definition, the use of TCA in concentrations of 50% or more, or in concentrations of 25-35% in combination with other acids in higher concentrations, such as glycolic acid, actually constitute a medium depth peel or even deeper peel. And the deeper the peel, the more the potential for serious and potential irreversible complications. Furthermore, the use of higher concentrations on non-facial areas, such as the neck, chest and extremities, carries that much more of a risk for the development of prolonged post-treatment hyperpigmenation and even permanent loss of pigmentation and scarring. The more coats applied the stronger the peel and the greater the potential for problems. Prolonged redness is usually a sign of potential complications. Chemical peeling is an aesthetic specialty unto itself and takes training and experience to perform properly and safely. I certainly understand the allure of do-it-yourself home treatment to save time and money, but playing with strong acids on one's own is literally playing with fire. Let this serve as a warning to anyone else reading this column contemplating self-treatment with strong peeling agents. Very important to look after this peel wound. Firstly prevention of infection, secondly prevention of skin colour changes, and thirdly the use of emollients like WSP , aqueous cream or aquaphore. Personally, I follow up all my TCA full face peels at least twice before discharge- once day one post op ( most patients have marked swelling), nursing follow up day 4, and discharge at day 7-9.
