I'm 36 and my general dentist offered a refund after admitting that he didn’t review my case before sending impressions on to Align. But 4 days into tray 3, the worst pain began in my back molar (crowned and all teeth missing around it). Pain intensity has steadily increased into week 2 of tray 3. I never opt for dental gas and only took Tylenol for multiple extractions, but I’ve been taking Aleve 24/7 for this throbbing pain. What could cause pain so severe in only one tooth? Loose crown?

Your xrays show that you had some very large fillings on your  back teeth, meaning you had very large deep cavities earlier in life.  I doubt this has anything to do with invisalign because I bet those teeth are not really being moved much to cause orthodontic pain.  Sounds like a pulp dying.  Get to the dentist.

There can be a number of sources for your extreme tooth pain--if the impressions were not accurate this could cause fit issues that cause too much torque on the tooth, if the tooth is being uprighted the bite may get to severe and start injuring the tooth, if a tooth has some pre-existing issues such as a crack or a non symptomatic infection these can also flair up during tooth movement. If the earlier aligner relieves the symptoms go back to wearing that stage and see your clinician. Good luck.
