I have had xanthelasma under my eyes removed by surgical excision twice in the last year and a half and both times it comes back within months around the scar. I feel like the surgeries made them angry and they grew. They are flat but make my eye look wrinkled and old (I’m only 38.) I have also lipids and a whole lot of other stuff checked. My ldl was a little high but cardiologist said not numbers he sees xanthelasma in. What can I do? My self esteem is tanking.

A mid facelift can do this. The skin is pulled posteriorly. You may also benefit from an extended browlift. This would lower your hairline and pull up the eyebrows. However, it would pull the skin around the eyes more posterior.
