I am 44 years old and have struggled with cystic acne for about 5 years. I only get one or 2 every other month, but I scar very easily. My dermatologist has tried topicals, antibiotics and years of Spironolactone. She feels Accutane is the best option. She says that most patients experience 2 weeks of purging. I am not aware of any studies evaluating this issue however often when treating adults, we tend to use lower doses because the visible side effects are not generally very acceptable to working adults. Meaning the dryness and peeling. And at lower doses most side effects are less likely.
