For the past few years, I have been looking into chin liposuction. I have always been a little self conscious of the fat around my chin. I searched through a variety of b/a pics, some changes looked dramatic, others were less so. My equation is: When it comes to the chin, why is it that some procedures couldn't suck ALL the fat out around the chin and looked as if there was still fat left? I know lipo is more complex than it seems, so I'm not trying to be unrealistic, I am just curious.

Answer: Fat removal in the neck with chin liposuction The anatomy of the neck is very complicated than what most people realize. There are 2 locations of fat in the neck. The first location is sub-mental fat which is directly below the skin on top of the platysma muscle. This fat can be removed with liposuction by either open or closed techniques. The second location is the sub-platysmal fat which is located below the platysma muscle. This fat is encased in dense fibrous tissue and must be surgically removed. Once the sub-platysmal fat has been removed, a platysma-plasty is performed to give a better shape and contour to the platysma neck muscles. This is considered a neck lift, but no skin has been tightened. When there is loose and extra skin present, a face and neck lift is required. Answer: What determines a good outcome There are two components to a great outcome in cosmetic surgery. The first is the patient being a good candidate. What this really means is that the diagnosis is made correctly and is correctly treated. For example, if someone really needs a chin implant but ends up getting liposuction, the results will be limited.Second is, of course, that the procedure is done correctly. My personal belief is when results are suboptimal, it has more to do with the first components rather than the second.Given enough assertiveness, every version of liposuction can remove virtually all fat in the chin area. This includes going beyond what is appropriate, leading to disfigurement. There is a saying in cosmetic surgery, "The enemy of good is perfect."Caution is a virtue.Best of luck, Mats Hagstrom, M.D.
