Can I take a second round of Accutane for small red bumps (1-3 everyday) and intense oily skin?

I took accutane 2 years ago and my skin was clear and mild oil for the 1st year. I took accutane for 7 months (1-2 mo 40mg, 3-4 mo 60mg, 5-7 mo 80mg). However, in the 2nd year, my skin got EXTREMELY oily to the point where I feel like washing it ALL THE TIME. It looks so bad. In addition, I get 1-3 small red bumps/whiteheads each day. Lastly, I use 0.1% Retin-A every night, but still nothing. It’s really bothersome. Do you think a second round of accutane will help? I am a 26 year-old female.

Acne requires a combination approach to get long term improvement.  For all patients on isotretinoin, we require them to be on skin care such as vitamin C, glycolic cream, kojic acid serums in the morning and retinols and salicylic acid at night.   All of our patients get clear+brilliant non ablative laser along with a microsecond YAG laser such as aerolase or laser genesis and microneedling with PRP or microneelding RF like intensif or fractora monthly to help improve breakouts, speed the healing process, and improve scarring and facial redness while on the isotretinoin/accutane.  LED light therapy, cosmelan/enlighten peels, salt facial or dermalinfusion can be done as well to speed up the improvement. Thank you for your question.Another course of Accutane might be beneficial, but unfortunately, acne may still recur. In some cases, a topical antibiotic and retinoid may be used for maintenance therapy after Accutane.
