Fat/volume loss in my upper eyelids - Am I good candidate for upper eyelid filler? (Photos)

For some background, I'm a 27 yo female. Around 3–4 years ago, I used an eyelash growth serum that I believe gradually caused some fat/volume loss in my upper eyelids. I've been off the serum for years now and haven't noticed any of that fullness come back. I know it's probably not something most people would notice, but it's something I've become a little insecure about. My eyes used to have a naturally hooded look that I really liked, and now they seem a bit more hollow or sunken than they used to. I also feel like it slightly brought out my negative cantal tilt and overall making me look more tired. I was wondering if upper eyelid filler would be a good option to restore some of that lost volume, or if there's another treatment that would be better

From what you've described, it's certainly possible that you're experiencing upper eyelid hollowing, and your observation about the lash serum is understandable. Prostaglandin analog-containing eyelash serums have been associated with periorbital fat atrophy, which can make the upper eyelids appear more sunken or less hooded in some individuals. While this effect may improve after stopping the product, it doesn't always completely reverse.

For someone your age, upper eyelid filler can be an effective option, but it should be approached with caution. The upper eyelid is one of the most technically demanding areas to inject because the skin is thin and the anatomy is delicate. When performed appropriately in carefully selected patients, a small amount of hyaluronic acid filler can restore volume, soften the hollow appearance, and recreate some of the fullness that was lost. However, overfilling can make the eyelids look heavy or unnatural.

There are also other considerations. If the volume loss is mild, conservative filler may be sufficient. If there is significant fat loss or changes affecting the surrounding brow and upper eyelid complex, procedures such as fat grafting may provide a longer-lasting solution, although they are more invasive.

Based on your description, you sound like someone who may be a reasonable candidate for upper eyelid filler, particularly if the primary concern is subtle hollowing rather than excess skin or eyelid laxity. The key is using a conservative approach with an injector who has extensive experience treating the upper eyelid area.

At 27, preserving a natural appearance is especially important, and subtle restoration often produces the most satisfying results. The goal should be to restore the soft contour you remember rather than adding noticeable volume.
