Hello! I am 21 years old and lost around 80 lbs. I’ve been to 3 consultations for my skin removal, and at two of which I was recommended a fleur de lis tummy tuck based on the fact that I want an hourglass shape. The scar is a lot to commit to, but I’m willing to trade the scar for a better shape if it’s needed! I was also told that I do not need lipo because I do not have fat to remove. Is this true as well? I do not have much loose skin in my lower back or buttocks area.
Answer: Wait, and perform conventional tummy tuck first Hello, I am Dr. Salvador Garcia from Guadalajara Mexico, I recommend you first perform the conventional or extended tummy tuck, and wait for how much your skin sticks, and then you decide if you like your result with the little sagging that could occur. stay or if you really have to complete the fleur de liz, in my experience with my patients, the majority decide do not to perform fleur de liz and they love their result. Answer: Fleur de Lis Thank you for question. In fleur de lis surgery, both a vertical incision in the midline and a horizontal incision in the lower abdomen are made. In this way, since the excess skin is removed both horizontally and vertically, the waist can be formed more clearly. However, the vertical surgical scar never heals as well as the horizontal scar. Sometimes it can form a hypertrophic scar. After an incision made only in the lower abdomen, waist thinning can be achieved by performing a good abdominal muscle repair, and extending the lower abdominal incision laterally will prevent skin accumulation in the midline.

Answer: A lower body lift would likely be your best option to avoid the vertical scar Judging by your pictures, you probably have excess skin going all the way around your torso, and a normal tummy tuck wouldn't address this. Although you may not have as much excess tissue along your lower back as your abdomen, you likely do have some, and if you had a lower body lift that goes all the way around your abdomen, you could address your excess abdominal tissue along with the tissue along your sides and back top avoid the vertical scar. I think you could get a great result with a lower body lift.

Answer: Considering Fleur-de-Lis Tummy Tuck for Hourglass Shape Thank you for sharing your history and photos, and congratulations on your significant weight loss. To achieve your desired hourglass shape, a vertical incision, such as in a fleur-de-lis tummy tuck, would likely be necessary. However, some surgeons may recommend a high lateral tension abdominoplasty, which gives a more defined hourglass contour than a standard tummy tuck. This procedure stretches the skin downwards and outwards over the hips. If the desired shape isn't achieved, a lateral incision could be made during a second surgery, offering a potential alternative to the central scar of a fleur-de-lis procedure. It's essential to weigh the pros and cons of each method, considering scar placement and the potential need for a second surgery. Regardless of your choice, I hope you achieve the look you're aiming for. Sincerely, Dr. Katzen.

Answer: Tummy Tuck, Liposuction "Congratulations on your impressive weight loss! Based on your description and the photos provided, it seems an extended tummy tuck might suffice to achieve the desired contouring while minimizing scars. The fleur de lis tummy tuck is typically recommended for significant central excess skin. If you don't want it, the extended variant could provide the hourglass shape you aim for. Regarding liposuction, if you've been consistently told you lack the excess fat, it might not be necessary. Always work closely with a board-certified plastic surgeon to determine the best approach tailored to your needs. Best of luck! "
