I typically keep the breast incisions taped which will support the skin while it heals, and provide some protection against rubbing. If garments are still painful, I recommend changing the garments, or providing extra padding. I imagine that is what your surgeon will recommend also. Best of luck!
Speak with your surgeon. He or she may want you to stop wearing the garment for a while or place gauze under the places where it is rubbing. Every surgeon has his or her own post op protocol. Best and sorry that you are in pain, Dr. Nazarian
You do not want a bra rubbing against an incision because this can cause a wound breakdown. Pad the area or get a new bra.
Sorry that you are having pain from the garment. Absolutely call the plastic surgeon and they will advise you to either wear a different bra or add some padding so it doesn't disturb your incision.
You appear to have a significant amount of nipple asymmetry at this stage and while you may see some improvement over the coming months as the swelling resolves, its likely you will need revision surgery to achieve symmetry. You will need to discuss your options with your surgeon....
