On the very top of my philtrum there's this slight bump that doesn’t cause me any pain, but can be contracted and uncontracted. My upper lip and be pushed more forward as I move this part of my lip. This part of my face has the most muscle tension and I often get muscle twitches on this part of my face as well. Any solutions would be appreciated

Attempting to surgery alter the philtum would be deforming and likely turn out worse. Personally, I find your lip attractive and would suggest doing nothing. You have very nice lip elements. However, if it bothers you I agree with the approach that you could inject around the philtral columns...
