How to know if a cohesive silicone implant is ruptured. What are the signs?

Unlike other silicone implants, cohesive gel implants do not typically leak into the surrounding tissue - it’s known as a “silent rupture” and most often, you won’t have any signs or symptoms associated with it. The “salad oil sign” you refer to on the MRI results can indicate that there is moisture or gas inside the implant, but that does not mean you’re in danger of a rupture. An MRI is the most accurate way to diagnose a rupture, so from what you describe, it sounds like you are fine. If you are concerned, regular MRIs to monitor your implants is a way to put your mind at ease. Even if you did experience a rupture, you would be able to tell by changes in the size and shape of your breast, as well as redness or inflammation in the area. I’m sure you also know that there is no evidence that silicone from implants causes cancer or any other serious health issues - your surgeon will most likely remove the implant and the leaked silicone and put in a replacement. Unlike older generation implants, if a cohesive silicone implant does rupture, it does not typically leak into the surrounding tissues Most often, patients have no idea that they have an implant rupture. If you are worried about an implant rupture, your plastic surgeon or PCP can order an MRI. The MRI is the most reliable diagnostic tool for a rupture today, so if the results show no rupture, that is likely to be accurate. I recommend you talk to your original surgeon or another board-certified plastic surgeon OB/GYN if you are concerned. I wish you all the best.

is still most reliably done with MRI, though US can certainly suggest its presence.  Clinically, if you do not have capsular contractures, I have found complete natural breasts (without appreciating the implant on displacement) is a reliable way to detect a rupture.
