I recently posted a question and would like to reask it since i forgot to add more. I spoke with an awesome surgeon and he pointed out the fact that i had a wide sternum. My concern for the surgery is that my breasts will be those type of breasts that are nightmares and are too far apart, however as long as I can wear a bikini and bra that can create the cleavage I want I will be happy. Is it possible or will i always need a bra that pushes my breasts up? will i always need a halter type of bra?
Answer: Cleavage Hello,Thank you for your question. I would suggest that for the most part, the anatomy/spacing you begin with is what you will end with via a standard BA. The space between your nipples will not change and your implants must be centered under them in order to look natural and ideal. Your Plastic Surgeon will likely measure your breast width carefully and select implants which fit your frame well, not too narrow, not too wide. It isn't possible to put them much closer together than your muscle and nipple placement will allow. I recommend that you seek a board certified Plastic Surgeon who's before and after photos you can relate to and ask lots of questions at your consultation regarding your expectations and aesthetic goals. All the best

Answer: Wide sternum Hello, I agree with your surgeon that your implants need to be placed properly under your breast tissue. It might be possible for the surgeon to narrow the gap between the breasts a little, but if they put the implants too close together the inner part of the implant will not have sufficient tissue coverage.

Answer: How far apart will my breasts be after surgery Hi and thanks for the question. This is a common concern for patients. You are correct in that your anatomy will determine your outcome. Patients with wider sternums do end up with more of a gap between the breasts. But that doesn't mean you can't achieve a pretty result. The gap can be reduced to some degree, but must be done judiciously. Like most patients we see, one of your main concerns is wondering how you are going to look after your procedure. My suggestion is to find a practice with the Vectra 3D imaging system. We use this to show our patients what they will look like with different sizes and styles of implants. We can also show you how your breads will change if we move the implants closer or further from your midline. Hope this helps. Good luck to you. Dr West

Answer: RealSelf . Breast Augmentation/Cleavage: Information Dear Ms. Denisenicole07,Thank you for your questions.I am glad you have a good experience during your consultation.Unfortunately without seeing nude photos it is difficult to comment on you specifically, however hopefully find the following information helpful.Cleavage in largely based upon your anatomy:1)the type implant placed may enhance the fullness centrally 2)If your chest is more rounded usually the breasts will rotate laterally (toward outer chest)3)if you are born with breasts close together more likely they will remain close togetherafter augmentation and vice versa.SYNMASTIAA complication which occurs when the central chest skin elevates and reduces cleavage. Often looking like and referred to as "unibreast" or "bread loaf" deformity. While this deformity can be corrected it requires additional surgery and expense.SUPRAPECTORAL PLACEMENTCleavage is dependent on the judgement of the surgeon on how close he/she can dissect without causing synmastia.SUBPECTORALCleavage is dependent on where the medial (central) origin (location)of the pectoralis major muscle is located.Improving cleavage can enhanced by manually movement of the breast implants centrally in the first few weeks after surgery to stretch the muscle and before the capsule (tissue around the implant) matures.PROFILE OF IMPLANT1)for the same volume the higher the profile the narrower the base width.2)there is very little difference in projection between a moderate to high profile implantthus little affect on how the actual nipple will look.3)the base width of the breast should equal the base width of the implant 4)thus I personally chose the profile based on the patients chest measurements, (a high profile on a wide chest may not result in the cleavage desired and conversely a low profile on a narrow chest may result in implant in the outside armarea).I would discuss your questions with your Plastic Surgeon who I trust is Certified by the American Board of Plastic Surgery and who is ideally members of the American Society for Aesthetic Plastic Surgery (denoting by membership as having met additional criteria and a focus on Cosmetic Plastic Surgery). My best wishes, R. Answer: Will my breasts be too far apart? Wide sternum causing concern for desired cleavage. Keep in mind that following the advice from a surgeon on this or any other website who proposes to tell you exactly what to do and what it will achieve without examining you, physically feeling the tissue, assessing your desired outcome, taking a full medical history, and discussing the pros and cons of each operative procedure would not be in your best interest. I would suggest that your plastic surgeon be certified by the American Board of Plastic Surgery and ideally a member of the American Society for Aesthetic Plastic Surgery (ASAPS) that you trust and are comfortable with. You should discuss your concerns with that surgeon in person. Robert Singer, MD FACS La Jolla, California
