Should I get a buccal fat removal? Are my edits realistic? (photos)

Young patients who are thin ( normal weight/ normal BMI) who have chubby cheeks and good cheek bones are usually excellent candidates for buccal fat removal the procedure allows the patient to have a thinner chiseled face, specially when smiling. The video below helps to understand who could be a candidate for buccal fat removal.

Great question!  Depending on your goals, a buccal lipectomy can help improve your facial shape by shrinking down the lower cheek region.  By doing this, the jawline and the cheeks become more defined and contoured.  Another treatment to think about is Botox to the posterior jawline as this will also help slim the face from a more squared to a more oval appearance on the frontal views.  However, it is always best to have an in person consultation with a board certified plastic surgeon so that your goals are reviewed and the best treatment recommended.

The buccal fat pad resides immediately below the malar (cheek) eminence. Removal of the fat pad will narrow the round face. Although the photos are somewhat excessively blurred (making it impossible to determine relationships with nearby structures), I think it would be worthwhile to visit a board certified plastic surgeon for in-person evaluation and advice.

The buccal fat pad is a rounded mass of fat in the middle of your cheek. It’s located between facial muscles, in the hollow area beneath your cheekbone. The size of your buccal fat pads affects your face shape.
Everyone has buccal fat pads. However, the size of buccal fat pads can vary greatly.
If you have larger buccal fat pads, you might feel like your face is too round or full.
You can be a candidate for a Buccal Pad removal surgery.

If you look at most before and afters you see the difference in a non-smiling view. While you may get a difference in the smiling view there is no guarantee of that so I would not do the surgery if that is your primary goal. In addition, for your flat cheeks you can augment them WITH the buccal fat (fat grafting). This lasts longer than filler and its already removed so ideally you shouldn't have to pay a crazy amount extra. 

Your edits are not realistic. Buccal fat removal does not affect the cheek area you have marked as that lies below where the buccal fat pad sits in the cheek. You would need a combined buccal fat removal and perioral liposuction which treats the whole cheek down to the jawline.
