Hello, and thank you for your posting. Usually accutane is dose anywhere between 0.5mg to 2mg/kg a day. I believe that you may be slightly underdosed, which is commonly done to avoid the drying and other side effects which patients don't tolerate at the beginning. . Have a discussion with your dermatologist to push the dose higher to 2mg/kg. If you are experiencing perimenstrual flares, consider discussing adding Spironolactone. Usually in my clinical experiences famales a bit slower to respond than men - it may have to do with the distribution of sebaceous glands. If I am concerned about absorption, and your insurance covers it, I may try to switch you to brand name Absorica. Newer studies suggest that the total cumulative dose should be 220mg/kg for the entire course, so you may not be quiet there yet. It's not uncommon to break out during the first 1-2 months. Talk to your board certified dermatologists about your concerns.
