I'm a big C cup and I'm happy with my cup size, I don't want implants but I don't want them to change size. Is it true that they look smaller after a lift? Or they appear bigger and "full" from being perkier, without actually increasing cup size?

Assuming your breast lift is performed by an experienced and talented breast surgeon, it is possible to achieve both an improvement in shape and the appearance of fulness as the breast tissue is redistributed over the anatomically correct (smaller) footprint of the breasts.

Answer: Breast lifting and Breast Size Change?

It is hard to promise or predict final cup size after breast surgery. With breast lifting, even though no breast tissue is removed, the breast shape and position are changed. This change in position and shape may result in a different cup size or a different level of filling of the cup of a bra. For example, a breast that sits lower on the chest wall may require a larger cup size bra than the same volume breast sitting higher on the chest wall. In my experience, patients who undergo breast lifting surgery, are pleased with the shape/position/perkiness of the breasts but often feel that they are smaller.
Therefore you should not base your goals or the success of your operation based on a cup size which may vary depending on who makes the bra. You should also be aware that you will not reliably achieve “upper volume” with breast lifting surgery (without the use of implants).
If you decide to proceed with surgery surgery it will be very important to communicate your goals with your surgeon. In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. I have found that the use of words such as “natural” or “C cup” or "fake looking" or "bigger" or "perkier" means different things to different people and therefore prove unhelpful.
