It is difficult to offer you a precise surgical plan without knowledge of your original surgery. Often, revisionary breast surgery is as much an art as it is a science. After all, the breast is an organ comprised essentially entirely of skin, soft fat and gland, and breasts come in endless anatomic configurations even before surgery! 
That being said, the combination of dog ears and "unfavorable" scarring are commonly encountered after breast reduction. Addressing dog ears can be done simply by excising them and resuturing the incisions, which typically end up slightly longer. This can often be done under local. As for the invaginated area beneath the areola, your surgeon will likely reopen the incision and mobilize, or rearrange, the soft tissue beneath to "rotate" it into the hollowed-out area after excising or otherwise disrupting any scar he may encounter.
