I had prophylactic, skin/nipple sparing double mastectomy with sailin implants in 2006. Complications and infections followed. right implant removed for 1 year. 2008 had DIEP flap with good results. lost 50 pounds and unhappy with the look of upper pole of breasts. Chest is flat down to the fuller flap portion of breasts. Is it a reasonable expectation to have an implant put in to fill the upper pole of the breast? Would it be best placed under muscle or over? I had fat grafting 85%is gone.

This is usually placed below the flap and above the pec muscle. I would suggest a round implant with a smaller diameter. The reason for this small diameter is so that the implant does not compromise the pedicle (blood vessels) of the DIEP flap. Even after 4 years your flap may still be reliant on the blood vessels to the flap. Contoured or anatomic implants will likely rotate in this sub-flap pocket as it is not a strongly adherent region. Also would suggest revisiting the idea of fat grafting the upper pole as it is generally a successful operation.
