I had 0.5ml of Juvederm injected into my lips 1.5 years ago. This summer, my injector gave me four separate rounds of hylenex, but my lips (the top in particular) remain bumpy when I smile. I am not sure if more hylenex is appropriate or if I need to seek out a specialist recommendations in MA welcome! Who can assess the nature of the soft bumps and determine if a steroid injection (for scar tissue/granuloma) or laser treatment is suitable. What do you think the bumps are? Any recommendations?

That is not the recommended practice. You should ideally consider removing/dissolving the fillers if they are displaced into an area they were not meant for. Steroids reduce swelling due to edema/inflammation. Not volume excess.
