Breastfed 1 child. Does 450cc (gummy bear) seem logical to obtain a D/DD size? (photos)

breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

Breast implants are sized according to their volume in cubic centimeters (cc).

How do you know what size implant to choose? Looking at pictures and talking to friends who had a breast augmentation might help.

Off course the volume will depend on the shape and size of your actual breasts, your frame and the size you want to achieve.

Our plastic surgeon will advise you thoroughly. Bringing pictures of a desired restult to be shown to our surgeon, might be very helpful.

Among all the tools available to predict final size after breast implants, my patients think sizers remain the most reasonable. Adding a small amount to that sizing amount is reasonable for submuscular placement. The average size implant placed in the US is 350 cc. Therefore, most reasonable implant sizes are from about 250 cc to 450 cc. The implant can be chosen to fit the size of the chest wall using one of several profiles. No relationship exists between the number of cc's and bra cup sizes. I would leave it up to your board certified plastic surgeon in the OR to select the best option for you based upon his/her experience, training, education, and aesthetic talents.

I think that sounds like a very reasonable size selection, but I would consider a donut mastopexy at the same time to lift your nipple position and tighten your skin. Your nipples are very widely spaced and they will appear even further apart if that issue is not addressed.

Breast implants will be inadequate to correct your problem.  Your breasts are too low on the chest wall and asymmetrical in volume, shape, position and widely separated.  You need a breast lift more than you need implants.  The technique I recommend is The Bellesoma Method.  This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage.  Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained.  Later, fat transfers can be performed if additional volume is desired.  Implants are not lifetime devices.  8% fail within the first year and 30% fail within six years.  Fat transfers are permanent and incorporated into the body.Best Wishes,Gary Horndeski, M.D.

Great question. The short answer is that it is impossible to tell you from the information andpictures you provided if 415 cc is a good plan for you. You bring up one of the hardest conceptsto overcome in breast surgery – cup size. Cup size is a very inaccurate measurements systemsin that it doesn’t apply to all body types, skin types, and breast composition. It does notmeasure breast surface area and breast volume, it simply measures a difference between thechest circumference below the breast and around the breast. In other words, cup size attempts to measure breast projection not volume. The problem with this is that projection is affected by more than just volume. For instance, if I put a cup of flour in a snack-sized Ziploc baggie and lay it on the counter next to a sandwich-sized Ziploc bag with a cup of flour and a gallon-sized Ziploc bag with a cup of flour, the smallest Ziploc bag will project above the countertop the most. Although they all have the same amount of volume, they all will have different projections or cup sizes. This is why it is so difficult for most women to get proper bra fitting (and why I think I would be an amazing bra fitter) and why bras from different companies seem to fit differently. The volume of a 34 C cup at Victoria Secret is going to be different than the volume of a 34 C bra from Hanes even though they have the same projection. The shape of the chest wall can also affect cup size as well as how much fat vs breast tissue the breast is made of.
During my breast enhancement consultations, I explain this concept and then we never talkabout cup size again. We get a measurement of the breast base width (or footprint) and thensimply talk about shape and projection compared to body type. Once the footprintmeasurement is known, this narrows the choices of implant sizes to just a few depending onhow much projection someone wants and how much their soft tissue can tolerate.I would recommend that you visit an experienced board-certified plastic surgeon who will beable to demonstrate all of this to you and take the appropriate measurements. The minimummeasurements they should take would be the width of each breast, the length from the top ofthe sternum to the nipple, the distance between nipples, the distance of the nipple to thebreast crease, and a pinch thickness of the skin of the upper pole. They may recommend abreast lift to one or both of your breasts as well which is common after breast feeding. Ratherthan talking about cup size, it is a great idea for you to provide pictures to your surgeon ofbreast size and shape that you desire. They can then let you know the possibilities. Once youhave you procedure done and are happy with your size and shape, you can go out and get newbras that fit well and don’t be worried about the cup size on the tag.

I prefer to take measurements, but everyone has their own way of doing things.  Just looking at your photos, I think that something in the 450cc range would be reasonable to get you where you want to be.  It's always a good idea to get a few opinions as you are doing and go with whomever you feel most comfortable.  Simulators are somewhat helpful to some in choosing a size as well.

 I usually recommend that my patients stop smoking, take vitamin C daily, and avoid aspirin/ibuprofen and vitamin E 3 weeks prior to surgery.
