Hello Drs. Thanks for your time . I'm 5'7 . 180lbs . Healthy . Non smoker. I had my initial breast implants in 2015. Had capsular contracture removal of the left (problem nipple) in 2016. The nipple was clipped and lopsided ever since. Or more noticeable. Fast forward 2023 I have courage to try again . Had implants exchanged from saline 545 cc to silicone 540 cc and fat transfer to try to balance nipples. Still lopsided . Is nipple reposition my fate ??

Answer: Scar tissue under areola is causing the depression under the breast
You did not post a photo, so I suggest you go back to the surgeon you used and ask if the areolar tissue is remaining and if it can also be removed. Sun exposure on a fresh scar can also turn it brown.
Based on this photo, it is very likely that excision of the scar and streching of current areola may create satisfactory improvement. In that case, a contralateral areola, if it is significantly larger, can be reduced in size. Grafting and tattoo usually do not create satisfactory result for...
Montgomery glands are intricate part of areola anatomy. Their excision would create scars that are at least twice as big and usually unacceptable. Think twice before doing it.
