Is nipple repositioning the only option to correct my uneven and lopsided nipples? (photos)

Hello Drs. Thanks for your time . I'm 5'7 . 180lbs . Healthy . Non smoker. I had my initial breast implants in 2015. Had capsular contracture removal of the left (problem nipple) in 2016. The nipple was clipped and lopsided ever since. Or more noticeable. Fast forward 2023 I have courage to try again . Had implants exchanged from saline 545 cc to silicone 540 cc and fat transfer to try to balance nipples. Still lopsided . Is nipple reposition my fate ??

Sometimes doing a breast implant surgery through the areola multiple times can cause a build up of scar tissue and the nipple can get pulled down and impede the "under breast" from rounding out. Even fat transfer can fail in these situations. If both of your breasts are soft and free of any capsular contracture at this point, the only way to correct the nipple position is to do a small lift, a lollipop scar, to raise the nipple and improve the appearance of the flat area under the current scar. You will have to do this on both sides for symmetry.

Hello;As I understand, breast implant exchange has been performed, but a lift (mastopexy) has not been done. Because of this, the areola-nipple complexes could not be elevated to their proper position, resulting in a sagging appearance. Additionally, due to adhesions in the semi-circular incision of the left areola, there is retraction in the scar. This cannot be corrected with fat injection. You need to undergo mastopexy.

Your breasts are too wide and too low on the chest wall. At this time, I recommend explantation with lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired.

Both of your areolas are lower than optimal on the breast mount. Periareolar lift, with a scar placed around areola would be the only way to change current nipple/areola position and scar adhesion on one side.. In person or teleconference evaluation with a board certified plastic surgeon with expertise in this procedure is recommended. Good luck.

Your areola can be positioned to sit a little higher with an incision all around the areola. Measurements are needed to see how much higher it should sit, It needed, both can be raised.

Thank you for your question and sending your pictures. You have a difficult problem and it appears your surgeon has tried to correct the asymmetry as best as possible. You probably need a small mastopexy procedure on your left breast and possibly on both sides.  You should contact your plastic surgeon to learn possible options
