Best technique for long lasting temporal brow lift?

Thank you for the important question. You have hit the nail on the head regarding the limitations of the endoscopic browlift. This is a very nice procedure, but the results are variable and unfortunately can be somewhat short-lived. This is because the procedure relies on suspending the tissue rather than removing excess. In my practice, I have prefer using an open approach to browlfting via a tricophytic incision (hairline). When this is performed meticulously, the hair grows right back through the incision, leaving no evidence of the procedure. And the major advantage is that this approach allows for removal of extra skin -- making the results much longer lasting. Best of luck moving forward!

Hello ozzThe specific procedure your brow lift entails will depend on your needs. Factors such as whether entire brow or only the outer brow is to be raised, repositioning of the hairline, and the patient’s individual facial contours will determine the placement and extent of the incisions.

The choice of browlift technique is individualized to each patient's anatomical situation.  In some cases, a temporal brow lift can be performed with relatively short incisions along the lateral frontal hairline.  More often, it is necessary to release the entire brow to stabilize it in a more refreshed and youthful position.  This can be performed through a hairline incision, or a series of endoscopic incisions.  In patients with a high hairline, I prefer a hairline incision because it actually shortens the forehead and lowers the hairline.  Best Wishes.

Most surgical brow lift techniques have results that last at least a few years. Generally speaking, transcoronal brow lifts -- also called hairline brow lifts -- tend to last the longest (up to ten years), but there are many factors that influence the longevity of your results other than the technique, including age, lifestyle and skincare practices.
It is most important to choose a technique that will accomplish the results you want while working seamlessly with your brow and facial structure -- when your brow lift works with your natural features, . You will be able to determine the technique that is right for you during your in-person consultation with your board-certified plastic surgeon -- if long-lasting results are your priority, be sure to tell your surgeon so they can consider this when making recommendations for your procedure. There are several different techniques for brow lifting, and this procedure is generally tailored to meet the specific needs of the patient. A typical browlift has two major components, a central release and a lateral release which addresses the entirety of the brow. Occasionally, a lateral lift is done in isolation through a temporal incision if the outer 1/2-1/3 of the brow needs to be elevated but the inner/medial part is in good position and no significant forehead wrinkles are present. Absorbable sutures are typically utilized to fix the tissue once its brought back and subsequent healing/internal scarring holds everything in place once the sutures resorb. The limitations of this procedure in isolation is that the brow can slowly come down over time but this is usually after several years. In my surgical practice, I typically favor an endoscopic approach to minimize visible scarring and post operative numbness. A consultation with a specialist in facial plastic surgery is recommended. Sometimes, brow position may actually be adequate and the real problem is volume loss and upper eyelid skin/fat excess. The type of Brow Lift  at least in part depends upon what you want to achieve.
The Endoscopic Brow Lift is good to remove the frown lines of the eye brows but does not improve the horizontal forehead lining. If this is combined with an elevation and excision of the temple scalp area "Temporal Male Brow Lift" the results are good and long lasting. To get good long lasting results the attachment of the brow to the bone of the upper eye area need to be fully released. 
If you need the entire forehead elevated and removal of the frown lines but also horizontal lines of the forehead  with a long lasting result then a Coronal Brow Lift would be best.

While many techniques exist for browlifting, an endoscopic techinque has become one of the more popular options due to the small incisions, no visible scarring, and shorter recovery time.  Regardless of the approach, a few techniques are important for a long-lasting result.-the brow tissue along the edge of the eye bone (arcus marginalis) needs to be completely released-after the brow is elevated, a good fixation to the skull bone with strong suture or endotines should be usedI recommend consulting with a facial plastic surgeon who has extensive experience in browlifting.  With these techniques, you can achieve a natural, long-lasting result.Best of luck!
