Fanonlyaa Thank you so much for your photograph. While it is helpful a complete exam is necessary to know your best course of action. Botox works by diminishing the strength of the muscle that has caused the wrinkle. The muscle we see on you is the frontalis but if we weaken this alone you are at risk of having your entire eyebrow fall. an expert injector needs to see all the surrounding muscles inaction to balance your upper face and give a natural result. May are under the false belief that anyone can inject Botox but it is very technique dependent. You have also lost volume in your forehead and this has caused this major wrinkle. I would suggest a combination of filler with the botox. My favorite filler to achieve this is artefill. I have been injection Botox for two decades and I am nationally certified Artefill injector. An in-person evaluation and examination of your dynamic facial expressions and static face is crucial. Wrinkles can be classified as dynamic (occurring with expression) or static (present at rest). If your wrinkles only form with expression, there is a good chance that Botox can nearly eliminate them. If, however, your wrinkles are static and deep (as you state in the photo), other modalities should be discussed with you for optimal results (e.g. resurfacing procedures or fillers). As long as you understand that Botox won't eliminate the static deep lines (just soften them), then you are in good shape. Having said all that, the average male dose in the glabella is around 25-30 units and around 10-12 units in the forehead. Some need more and some less. Your brows seem heavy in this photo, so a conservative approach is best initially, but again, the in-person exam is key. This stresses that only "core" injectors should perform cosmetic injectables (facial plastic surgeons, plastic surgeons, dermatologists, or occuloplastic surgeons). This is because we all have a keen understanding of facial anatomy and the effects of injecting Botox and fillers in different regions of the face. In general 20 units of Botox is required for the area between the eyebrows. For the forehead, I will usually use fewer than 10 units to avoid a "frozen" or over-treated appearance. Given the depth of these lines and the fact that they're folded into your skin you might, at some point, also benefit from a dermal filler to pop out the lines that is imprinted on the skin. Botox treatment needs to be tailored to the individual. Since I cannot see your eyebrow position or your eyes/eyelids, I cannot give you the best "unit" dosing for a natural result. Often men require 20-30 units in the glabella (between the brow) and 10-20 units on the forehead. If you have low eye brows to start, it is best to only do the glabella and a small dose high on the forehead to ensure you do not drop your brows or over "freeze" the forehead. The forehead typically is a much easier area for people to "spot" when over-treated. Your glabella will likely need at least 20 units for full treatment. Your forehead may have a nice outcome with 10 units initially, and more could be added on an as needed basis.
An injector typically injects the minimum amount of Botox initially. Botox takes 2 weeks to completely settle. After the 2 weeks you can schedule a follow up appointment to administer additional units if required. The line across your forehead may require a filling product in order to soften the line to desired look. All the Best
For a natural looking smoothening of the lines in the frown and forehead areas, typically between 30 to 40 units of Botox are needed. The frown area commonly requires around 20 units, and the forehead between 15 to 20 units. However, when treating men we may often use the higher amount and extend the injection close to the hairline as well, in order to treat the strong frontalis muscle adequately.
Hi fanonlyaa:10-20 units of Botox should do it for the mid-forehead line, BUT....You appear to have heavy brows and if you over inject, you may have a "frozen" forehead, working eyebrows and brows falling over your eyes. Since it takes 10-14 days to optimize your Botox results, you may wish to take "baby" steps and inject small amounts every 2 weeks, mapping your injections in your medical file. Then you can identify the balance required to elevate your brows but reduce the mid-forehead furrow. Overall, anticipate it will take more than the 20 units as you may need the inter-brow muscles and lateral brow muscles injected to get the lift of the brows to offset the descent of the forehead.You can now appreciate the art and skill behind the science. Botox dosing must be individualized in each patient in order to obtain a natural and balanced result. The dose of a neuromodulator (Botox, Xeomin or Dysport) will vary depending on the amount of muscle activity, size of the muscle and location as well as an assessment of the surrounding facial muscle activity and any possible asymmetry.In order to fairly assess your forehead, I would want to see the brow position and surrounding facial structures in order to precisely estimate the Botox dose.If there is any sag or drop of the brows or forehead tissues (ptosis), the Botox must be dosed and placed carefully in order to avoid further descent of the brows as the medication takes effect. Proper use of Botox below the brows can be effective in elevating the lateral or medial brow to reduce brow sag or ptosis.In general, the forehead may take 10 - 25 units and the glabella approximately 20-30 units of Botox or Xeomin depending on the above factors. It should also be mentioned that deep forehead or glabellar creases that do not diminish adequately with Botox or that are present at rest, can be effectively treated with injectable fillers such as Restylane, Juvaderm or Belotero. In our practice we strongly encourage patients to improve their skin health with medical skin care, proper exfoliation and chemical peels in order to optimize their results with Botox or fillers.
