Once hyaluronic-acid fillers settle, they integrate into the surrounding tissue. They don’t sit in a pocket that can burst open. Even “fresh” filler isn’t something that can ooze out from trauma the way people imagine.

That said, any strong hit to the mouth can cause swelling, bruising, or movement of the filler, just like it can affect your natural tissue. If the trauma is severe enough to break the skin or cause a laceration, then anything in that area—filler included—could be affected, but that’s due to the injury itself, not the filler leaking on its own.

Great question. Fillers that are used on the lips (usually Hyaluronic Acid) remains being a cohesive gel after injection. The body will produce a mild inflammatory reaction to contain it which result in a stable position of the gel while the body degrades it. So, YES, a significant trauma can shift the gel to a different position.
